How to get an amendment proposed:
In order for an amendment to be proposed it must obtain either a positive 2/3 vote in both chambers of Congress, or a national convention must be called at the request of 2/3 of the states.
How to get an amendment ratified:
The proposed amendment needs to then receive a vote by the legislatures of 3/4 of the states or a convention needs to be called in the states and the amendment must get a 3/4 vote from them in order for it to be ratified. The second method is through special conventions called in each state for the specific purpose of ratifying the proposed amendment.
